The association between HSG and pregnancy complications in patients conceived shortly after the procedure is controversial. Hypothesis Women who conceived shortly after an HSG are at increased risk for pregnancy complications due to exposure of the endometrium to the contrast media Objectives To assess whether a higher rate of pregnancy complications is associated with a shorter interval of conception after a hysterosalpingography (HSG) procedure Results A total of 2289 HSG exams and 1043 pregnancies after HSG were analyzed. There was an increase in preeclampsia in women conceived during the first six months after HSG compared with women conceived later than six months after HSG, with an incidence of 10.5% versus 5.9% (adjusted odds ratio [aOR] = 2.14 (1.06-4.3); 95% confidence interval. Similar incidences of miscarriage, gestational diabetes, preterm premature rupture of membranes were observed. There was no significant difference in gestational age at delivery, rate of preterm birth, low birth weight, neonatal intensive care unit admission or birth defects. All trends, including the significantly increased risk of preeclampsia in pregnancies conceived after HSG, persisted upon stratification of analysis to singleton live births. Conclusions To date, this is the largest study examining the association between HSG and adverse pregnancy outcomes. There was a statistically significant increase in the odds of preeclampsia associated with a shorter interval from HSG to pregnancy. A further prospective investigation is warranted.
