Description

 

The staff of the medical information and documenting department is responsible for the concentration of the medical information of the patient from all his visits at the Barzilai medical center: departments, clinics and emergency department.

Fourteen employees work at the medical information and documenting department, and each one is assigned for a special area.

Some of the medical information is still stored as printed documents but during the last 20 years all the medical documentation became digital. The digitalized documenting system has been improved so that when a patient comes in every hospital in Israel, all the medical history of the patient including his visits in the HMOs become available to the doctors, and vice versa. The family physicians at the HMOs can read the medical documents of the hospitals each time the patient comes in for a visit.

 

The medical information and documentation is important for the follow up of the patients but may serve the physicians for research, quality control, lecturing and for the administration for policymaking.

 

All the employees of the medical information and documenting department must sign a declaration that they will follow the law of confidentiality of the medical information.

 

Special Activities

 

r  Archiving all the medical information of the patient: departments, clinics, birth certificates, laboratory results and death papers.

r  Separating the documents of the differential surgeries for the financial department.

r  Mortality reporting to the Ministry of Health of unexpected deaths.

r  Mortality reporting to the Ministry of Health of deaths from malignant diseases.

r  International coding of diseases.

r  Delivering medical information to legal authorities.

r  Delivering medical information to risk management unit of the hospital.

r  Mentoring of the medical secretaries and the students of the medical secretary courses.

r  Delivering medical information during mass and war events.

r  Delivering medical information and documents to the families after informed consent of the patient.
